Petition Form

Students may petition their dismissal or suspension from the School of Engineering by completing this form in its entirety.   Basic demographic information and a statement concerning the extenuating circumstances must be completed by utilizing the form below.  

Once the petition form is completed and submitted it will be forwarded to the appropriate department advisor.  The advisor will determine if there is substantial information to forward to the committee.  If not, the advisor will inform the student and if time permits before the semester deadline, student can submit a revised petition.

Deadline following fall semester: Wednesday before spring classes start for suspended students.  Dismissed students can submit a petition through the end of the first week of the Spring semester. 

Deadline following spring semester: June 30th for suspended students.  Dismissed students can file a petition through the end of the first week of the Fall semester (August 25, 2017). 

Please be aware the form below must be completed in one setting.  It cannot be saved and re-started.
