Petition Form

Students may petition their dismissal or suspension from the School of Engineering by completing this form in its entirety. Basic demographic information and a statement concerning the extenuating circumstances must be completed by utilizing the form below.

Once the petition form is completed and submitted it will be forwarded to the appropriate department advisor. The advisor will review before forwarding to the committee for decision.

Deadline following Fall semester: Wednesday before spring classes start for suspended students (January 9, 2019). Dismissed students can submit a petition through the end of the first week of the Spring semester (January 18, 2019).

Deadline following Spring semester: June 21st for suspended students. Dismissed students can file a petition through the end of the first week of the Fall semester (August 23, 2019).
